Effective since January 1, 2003, all maintenance requests MUST BE submitted to Cournale & Co. in writing. Please complete this online form to submit your written request. All requests are subject to approval by Owner and/or Cournale & Co.

In the event of an Emergency, i.e. Fire or Flood, call 9-1-1.

*If the answer above is “No” then you understand that you will have to be available to provide the contractor access into the unit. Appointments generally require a four hour window. We will always knock before entering.

Note: (a) If resident has requested an appointment and is not present at the agreed upon time, the service call fee will be charged to the resident. Note: (b) If locks have been changed without Cournale & Co.’s consent, and a contractor is unable to enter the unit for the requested repairs above, the service call fee will be charged to the resident. By signing and returning (submitting) this form, undersigned acknowledges that they read and understood the information provided. Undersigned also acknowledges our future intent to enter your apartment, subject to the access instructions above. We may contact you with a more specific date and time; however, undersigned waives requirement to receive a written notice to enter unit in order to expedite service. Note: In an emergency situation it may be necessary to enter without any prior notice. *
